Reimbursement Hotline

Our Hotline is a full-service reimbursement assistance program that anyone can contact when they have benefit questions. Our goal is to make it easy to gain access to Erwinaze and provide patients with the help they need. Callers will recieve assistance with:

  • Coverage and coding support The Hotline is available to provide support to healthcare providers completing claim forms and other reimbursement documents. We provide guidance on payer coverage, coding and reimbursement policies.
  • Benefit Investigation The Hotline can perform benefit investigations and alternative coverage searches to identify what coverage a specific patient has. We will explain all the available options we can find for the specific situation of each patient. Is the drug covered? What will the patient owe? Is prior authorization required?
  • Prior Authorization Hotline reimbursement counselors will determine if prior authorization is necessary. If so, we will obtain the prior authorization forms and determine the exact process and timeframe that needs to be followed. Our counselors will pursue discussions with the healthcare provider and insurance payer throughout the process.
  • Appeals If the insurer denies coverage, our reimbursement counselors are available to assist the healthcare providers obtain the documentation to demonstrate the medical necessity of the patient's therapy. "Letter of Medical Necessity" templates as well as other appeal templates will be provided. Counselors will provide guidance throughout all levels of the appeals process.
  • Other services include support with alternative coverage, formulary restrictions, and reimbursement materials. To obtain assistance, patients and physicians must complete and sign this application form. All patient assistance applications are processed within two business days.

Selected Risk Information about Erwinaze™ (asparaginase Erwinia chrysanthemi)

CONTRAINDICATIONS: Hx of serious hypersensitivity to Erwinaze, including anaphylaxis; Hx of serious pancreatitis, thrombosis or hemorrhage with prior L-asparaginase therapy.

WARNINGS AND PRECAUTIONS: Discontinue if serious hypersensitivity, including anaphylaxis, OR severe or hemorrhagic pancreatitis occur. Monitor glucose (intolerance may not be reversible); With thrombosis and hemorrhage: discontinue until resolved. Use in pregnant women only if clearly needed. Do not use in lactating women.

COMMON (≥ 1%) ADVERSE REACTIONS: Serious hypersensitivity, including anaphylaxis, pancreatitis, abnormal transaminases, coagulation abnormalities, nausea, vomiting, and hyperglycemia.
